Order Form

Guidelines

1. Melissa will be compiling and sending out the order form, extras email, and final order details. Please email any item requests to freshorganics@embarqmail.com Kevin will be distributing the orders at Healthy Habits.
2. Order form will be sent out the 1st and 3rd Saturday of the month and posted on www.freshorganicsswfl.com
3. Orders must be received no later than Sunday midnight.
4. Once you place your order you are obligated to purchase the items you selected.
5. The order will be placed on Monday; if anything is out of stock and there is an acceptable substitute we will contact you regarding the change (your choice).
6. Items must be picked up between 3 and 6 pm on Wednesday unless prior arrangements are made.
7. Payments can be made by cash, check, or Visa/MasterCard/Discover (add 2.5%fee for credit cards).
8. There will be a $15 fee for returned checks.
9. Orders over $50 require pre-payment on Monday/Tuesday. We can keep your credit card number securely on file for your convenience.
10. If you order a full case you will receive a 10% discount.
